What is Pace?

Things you need to know about the PACE program
The Program of All-Inclusive Care for the Elderly (PACE) model of care provides a comprehensive medical/social service delivery system using an interdisciplinary team approach in a PACE Center that provides and coordinates all needed preventive primary, acute, and long-term care services. Services are provided to older adults who would otherwise reside in nursing facilities.

Are you eligible?

You are 55 or older. -
2.
You live in the service area of our PACE center.

You are eligible for nursing home care. -
4.
You are able to live safely in the community.

The cost of the program
If you are eligible for Medi-Cal, Our services are covered at no additional cost. Medi-Cal beneficiaries who have a share of cost remain responsible for their individual share of cost.
If you do not qualify for Medi-Cal but you have Medicare, you will be charged a monthly premium to cover the long-term care portion of the PACE Center benefit and a premium for Medicare Part D drugs.
If you do not have Medicare or Medi-Cal, you can pay for The PACE Center privately.
There are no deductibles, copayments, or coinsurance for any care and services approved by our team of healthcare professionals.
