Job Summary:
Under the supervision of the Executive Director, manages flow of clinic leading all staff in provision of medical and nursing care and collaboratively developing workflows and policies, ensuring high quality standards. Coordinates services with primary care team and acts as a primary liaison. Triages participants as needed as the gatekeeper for daily clinic scheduling. Provides skilled nursing care to participants; conducts assessments; implements provider orders; develops appropriate plans of care; provides education and training to participant, caregivers, family and staff; and communicates participant’s condition to caregivers, primary care, and the interdisciplinary team. Has direct oversight of all responsibilities for paraprofessionals (CNAs, CMAs, PCAs, HHAs, homemakers) including clinical education, training/observations, reporting requirements, and competency evaluation.
Qualifications and Requirements:
Education:
- Associates Nursing Degree required, BSN preferred.
- Registered Nurse license
- BCLS Certification required.
Experience:
- Minimum of 1 year working with the frail or elderly population.
- Two (2) years nurse management experience in a hospital, nursing home or community-based setting.
Skills and Knowledge:
- Skilled in the management of personnel and a department.
- Skilled in developing workflows and processes.
- Excellent organizational skills.
- Excellent written and verbal communication skills.
- Broad knowledge base of health care problems of the frail elderly.
- Skilled in physical assessment and triaging.
- Knowledge of common safety hazards and precautions to establish a safe work environment.
- Skilled in preparing and maintaining records and writing reports and responding to correspondence.
- Skilled in developing and maintaining department quality assurance.
- Skilled in establishing and maintaining effective working relationships with participants, medical staff, staff members and family caregivers.
- Ability to react calmly and effectively in emergency situations.
Medical Clearance:
Employees must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
Competency:
Position specific competencies for the Registered Nurse will be met prior to assuming participant care.
Physical Demands and Working Conditions:
The physical demands and work environment characteristics described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Work Environment:
Employee must be able to work effectively within an interdisciplinary team model, interfacing and collaborating with a wide range of clinical and social services disciplines who work together to manage the PACE participants’ care. The work setting is in an Adult Day Health Center and primary care clinic environment with moderate noise levels and controlled temperatures.
Physical Requirements:
Requires manual and finger dexterity and eye-hand coordination; the ability to use department equipment; and the ability to lift/carry up to 30 pounds using appropriate body mechanics with reasonable accommodation if needed.
Visual, Hearing and Communication Requirements:
Requires corrected vision and hearing to normal range, with or without reasonable accommodation. Ability to communicate by way of the telephone with participants, customers, vendors and staff.
Pressure Factor:
Requires working under stressful conditions. Working conditions may be noisy and crowded and fluctuating indoor temperatures. Moderate pressure to meet scheduled deadlines. Potential for exposure to verbal aggression by client, vendors, and staff.
Environmental Conditions:
May be exposed to a risk of bodily injury through contact with moving instrumentation, toxic substances, medicinal preparations, bodily fluids, communicable diseases and any other conditions common in a healthcare environment. Subject to unpleasant odors.
Duties and Responsibilities:
- Engage in team building. Coach and mentor staff. Instruct and teach staff as needed. Train clinic staff to function independently.
- Evaluate overall effectiveness of the clinic, implementing change and quality improvement as needed in collaboration with the Medical Director.
- Manage flow of clinic, oversee appointment bookings, triaging and gate keeping in conjunction with primary care and staff. Ensure timely and appropriate care of participants.
- Monitor clinic departmental expenditures and meet budgetary expectations.
- Communicate effectively with the Executive Director and the Medical Director to keep them abreast of any issues or concerns.
- Participate as a member of the interdisciplinary team, with regular attendance at, and active participation in, all interdisciplinary team and care planning meetings, communicating participant changes, collaborating on care planning decisions, and coordination of 24-hour care delivery.
- Provide orientation, initial and annual competencies, supervise and evaluate performance of Clinic RNs, Clinic Assistant’s, and Clinic Clerk.
- Provide direct oversight of the paraprofessionals (CNAs, CMAs, Pharmacy/Med technicians) in the clinic, paraprofessional clinical education, training/observations, reporting requirements, and competency evaluations.
- Educate staff in care and treatment of the elderly as needed.
- Ensure that the job responsibilities, authorities, and accountability of direct reports are defined and understood. Ensures that clinic staff works within their scope of practice.
- Take lead in effectively communicating with interdisciplinary team and community agencies at meetings and throughout the day.
- Oversee and ensure signing off orders, ordering medications from pharmacy and disseminating orders and instructions to appropriate personnel. Ensure implementation of provider orders.
- Coordinate participant care with outside contracted service providers – nursing homes, assisted living, lab, oxygen, etc.
- Communicate with weekend and after hour staff, following up appropriately.
- Manage and review participants’ records, charts, pertinent information, and staff’s documentation. Document as needed in medical records according to PACE standards.
- Conduct in person comprehensive initial, regularly scheduled, or unscheduled reassessments of PACE enrollees, according to program protocols and needs.
- Perform general skilled nursing care to participants. Perform or supervise the administration of prescribed medications and treatments in accordance with nursing standards. Assist in treatment, examination and testing of participants.
- Share rotating on-call responsibilities with all staff nurses.
- Respond to and perform triage of incoming telephone calls. Oversee scheduling or outside appointments for participants when necessary.
- Assist with all aspects of hospital, nursing home and other institutional admissions.
- Educate participants, family and caregivers in care, medications, treatments and all other aspects of health education as needed.
- Follow all PACE policies and procedures and OSHA safety guidelines.
- Protect privacy and maintain confidentiality of all company procedures and information about employees, participants and families.
- Participate in continuing education classes and any required staff and training meetings. Maintain professional affiliations and any required certifications.
- Participate in and support Quality Improvement initiatives.
- Act only within scope of practice.
- Perform other duties as required or requested.
