In Home Services Companion

Job Summary:

Under the supervision of the In–Home Services Coordinator, the Companion is responsible for providing and assisting participants with supportive care, limited to the duties and responsibilities described below, necessary to maintain the participant in a safe home environment.

Qualifications and Requirements:

Education:
  • High school diploma or equivalent.
Experience:
  • A minimum of one year’s experience in working with a frail or elderly population.
Skills and Knowledge:
  • Skilled in establishing and maintaining effective working relationships with participants, co-workers, and the public.
  • Skilled in identifying and recommending problem resolution.
  • Ability to work independently with minimum supervision.
  • Ability to communicate clearly and effectively.
  • Ability to work with a frail and cognitively impaired population

Physical Demands and Working Conditions:

The physical demands and work environment characteristics described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Work Environment:

Employee must be able to work effectively within an interdisciplinary team model, interfacing and collaborating with a wide range of clinical and social services disciplines who work together to manage the PACE participants’ care. The work setting is in an Adult Day Health Center and primary care clinic environment with moderate noise levels and controlled temperatures.

Physical Requirements:

Requires manual and finger dexterity and eye-hand coordination; the ability to use department equipment; and the ability to lift/carry up to 30 pounds using appropriate body mechanics with reasonable accommodation if needed.

Visual, Hearing and Communication Requirements:

Requires corrected vision and hearing to normal range, with or without reasonable accommodation. Ability to communicate by way of the telephone with participants, customers, vendors and staff.

Pressure Factor:

Requires working under stressful conditions. Working conditions may be noisy and crowded and fluctuating indoor temperatures. Moderate pressure to meet scheduled deadlines. Potential for exposure to verbal aggression by client, vendors, and staff.

Environmental Conditions:

May be exposed to a risk of bodily injury through contact with moving instrumentation, toxic substances, medicinal preparations, bodily fluids, communicable diseases and any other conditions common in a healthcare environment. Subject to unpleasant odors.

Duties and Responsibilities:

  • Scan home environment for safety issues and report any new hazards to their supervisor.
  • Assist participant with daily routine including:
  • Selection  of clothing
  • Making bed
  • Linen Changes
  • Caring for pets or plants
  • Ironing and mending of clothing
  • Organizing and reading mail
  • Medication reminders
  • Assists participants with meal planning, grocery shopping,  putting away groceries in appropriate areas, preparation of food and serving meals in accordance with dietary requirements.
  • Provides companionship, socialization, friendship and support, initiates games, crafts, reading, and similar activities as per plan of care
  • Provides  outdoor walks and exercise per plan of care
  • For cognitively impaired participants provide supervision and cueing for periods of caregiver absence.
  • Accompanying participant to shopping, social events, doctor and other appointments
  • Assists participant with housekeeping that is specific to the participants living area 
  • Light dusting
  • Light vacuuming of carpet
  • Sweeping and  mopping floors
  • Wash Dishes/load dishwasher
  • Participant Laundry
  • Take out trash
  • Documents services provided to participants consistently and accurately via approved system/Daily Activity Record.  Documentation is completed as per required time frames. Completes all participant documentation in the medical record per documentation standards.
  • Implements In-home services plan of care established by the Interdisciplinary Team (IDT); contacts supervisor for any questions or need for clarification of the plan or to report problems in implementing plan.
  • Reports pertinent information regarding participants in a timely manner to    appropriate team members and supervisor.
  • Participates in participants’ care planning through cooperation with interdisciplinary teams.
  • Performs other tasks as assigned by Director of In-Home Services /R.N as instructed (within scope of allowable practice).
  • Participates in and supports Quality Improvement Initiatives.
  • Follows al Policies and Procedures and OSHA safety guidelines
  • Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal precautions.
  • Maintains safe working environment. Follows Safety Policies and Procedures.
