As a PACE participant, you have access to some of the finest team of medical care professionals specializing in aging and chronic health conditions. Our doctors, nurse practitioners, registered nurses, therapists, dietitians, social workers, and home care nurses will become your team and together we will design a tailored health plan just for you, to keep you healthy and independent as long as possible.

In-Home Care
While living independently in your home you may find that you need additional support, we’re here to help.
Aside from the medical and health services we provide at our PACE center, we have a wide variety of programs to support you as you live independently at your home. When you enroll with us, our dedicated nursing staff will determine if in-home care would be helpful in supporting your current lifestyle.
Our trained and certified staff of in-home care professional will come to you to provide a wide variety of services, including:

Day Center
Our day center was created with an atmosphere of fun activities that promote social interaction among our seniors so that their visit is not just about medicine but also about community.
Having a sense of fulfillment and peace of mind is so essential in staying healthy that we designed our center and our programs to reflect that sentiment. When visitors walk into our day center, they are often struck by a feeling of walking into a spa rather than a medical facility. There are activities that promote social interactions that nurture the body and spirit, programs designed to stimulate the mind, and a safe environment where the need for companionship, friendship, and community is fulfilled.


Physical Therapy
Our physical therapy programs are individually tailored to help our seniors live independently in their homes.
As we experience the challenges of living our golden years, we often confront physical limitations due to the natural cause of aging, injuries, surgery, or illness. As part of your PACE health plan, our experienced professionals will design a comprehensive physical rehabilitation regimen to restore your mobility, reduce pain and increase your overall fitness level to promote your ability to live independently.
Transportation
We will provide transportation from your home to our center, making it convenient for you to get the care you need.
For seniors who can no longer drive, we understand that coordinating transportation to and from doctor appointments can be challenging and expensive.
Our dedicated drivers will pick you up from your home in the morning to bring you to our center and in the afternoon they will drive back to your home. While you are at our center, you can enjoy the company of others and participate in various social activities.
We also arrange fun-filled outings and field trips to beaches, parks, and others sites of interest all over Los Angeles and surrounding cities. Check our monthly events calendar for more information.


Meal Preperation
Nutrition and diet are a big part of keeping healthy and at our center, we have a team of Nutritionists who will prepare delicious dishes that meet the guidelines of Recommended Daily Allowance.
Our seniors will enjoy a menu of Asian and Western dishes along with fresh fruits and vegetables at our cafeteria. Also, our in-home care staffs are trained nutritionists and they will prepare nutritious meals as prescribed by your physician.
