Job Summary:
Responsible for coordination and scheduling of in-home services for all (PACE Program) participants. Prepares schedules according to participant needs, Director of In-home care requests, and within the plan of care designed by the interdisciplinary team. Works closely with the transportation department, clients, caregivers and center staff.
Qualifications and Requirements:
Education:
- High school diploma or equivalent.
Experience:
- One years of experience in working with the frail or elderly is required.
- Experience or training in health care delivery, in home health is preferred.
- One year or more of scheduling and/or dispatching experience preferred.
Skills and Knowledge:
- Skilled in establishing and maintaining effective working relationship’s with participants, co-workers, medical staff, and the public.
- Skilled in identifying and recommending problem resolution.
- Ability to work independently with minimum supervision.
- Ability to communicate clearly and effectively.
- Ability to work under tight time constraints and schedules.
- Ability to make decisions and prioritize to meet the needs of the participants.
- Ability to give clear and concise directions to in-home service staff.

The physical demands and work environment characteristics described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Work Environment:
Employee must be able to work effectively within an interdisciplinary team model, interfacing and collaborating with a wide range of clinical and social services disciplines who work together to manage the PACE participants’ care. The work setting is in an Adult Day Health Center and primary care clinic environment with moderate noise levels and controlled temperatures.
Physical Requirements:
Requires manual and finger dexterity and eye-hand coordination; the ability to use department equipment; and the ability to lift/carry up to 30 pounds using appropriate body mechanics with reasonable accommodation if needed.
Visual, Hearing and Communication Requirements:
Requires corrected vision and hearing to normal range, with or without reasonable accommodation. Ability to communicate by way of the telephone with participants, customers, vendors and staff.
Pressure Factor:
Requires working under stressful conditions. Working conditions may be noisy and crowded and fluctuating indoor temperatures. Moderate pressure to meet scheduled deadlines. Potential for exposure to verbal aggression by client, vendors, and staff.
Environmental Conditions:
May be exposed to a risk of bodily injury through contact with moving instrumentation, toxic substances, medicinal preparations, bodily fluids, communicable diseases and any other conditions common in a healthcare environment. Subject to unpleasant odors.
Duties and Responsibilities:
- Schedules in-home service staff appropriately to meet participant needs.
- Communicates staffing needs and ability to meet participant needs with Director of In-Home Service.
- Coordinates and communicates effectively with the transportation department to ensure smooth transition between home care and transportation delivery of services.
- Notifies participants/families of changes in their home care schedules.
- Prepares and maintains daily, weekly, and/or monthly (written or electronically) service schedules, logs, ledgers, tickler systems and other records that ensures accurate data collection of home care services.
- Communicates clearly with participants, staff and supervisor directly and in meetings.
- Assists with filing and other office functions as requested.
- When authorized communicates with outside agencies and independent contractors regarding services rendered on behalf of PACE Program.
- Attends Intake and Assessment meetings if requested by Interdisciplinary team or supervisor.
- Attends all departmental meetings.
- Reports pertinent participant events or missed participant in home service visits to Director of In-Home Services.
- Follows all PACE Program Policies and Procedures and OSHA safety guidelines.
- Maintains the confidentiality of all company procedures, results, and information about participants, clients or families.
- Practices Universal precautions and follows appropriate Infection Control procedures.
- Maintains safe working environment. Follows PACE Program Safety Policies and Procedures.
- Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.
- Performs other duties as required or requested.
