Day Center Worker
(Certified Nursing Assistant)

Job Summary:
Under the supervision of the Day Center Manager performs ADL’s and activities within the scope of practice of a Certified Nursing Assistant such as clinical observation, clinical reporting requirements, participant laundry, participant care training and education. It is the responsibility of the Day Center Worker for providing and assisting participants with activities of daily living, restorative and supportive care. Monitors health status and provides a safe environment for participants. Assists with organization and implementation of activity programs. Functions as a member of the interdisciplinary team on an as needed basis.
Qualifications and Requirements:
Education:
- Current CNA [State] Certification
Experience:
- Minimum of 1 year working with the frail or elderly population or training and tested competency provided by [PACE organization].
Skills and Knowledge:
- Knowledge of principles, practices, standards and techniques of a CNA.
- Knowledge of common safety hazards and precautions for a safe work environment.
- Basic knowledge in working with frail and elderly to support Activities of Daily Living and Incidental Activities of Daily Living.
- Ability to work independently with minimum supervision.
- Able to establish and maintain effective working relationships with patients, co-workers, medical staff and the public.
- Ability to communicate clearly and effectively.
- Ability to react calmly and effectively in emergency situations
- Basic computer literacy
- Ability to work effectively with culturally, economically and educationally diverse populations.
Medical Clearance:
Employees must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
Competency:
Position specific competencies for the Certified Nursing Assistant will be met prior to assuming participant care.
Physical Demands and Working Conditions:
The physical demands and work environment characteristics described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Work Environment:
Employee must be able to work effectively within an interdisciplinary team model, interfacing and collaborating with a wide range of clinical and social services disciplines who work together to manage the PACE participants’ care. The work setting is in an Adult Day Health Center and primary care clinic environment with moderate noise levels and controlled temperatures.
Physical Requirements:
Requires manual and finger dexterity and eye-hand coordination; the ability to use department equipment; and the ability to lift/carry up to 30 pounds using appropriate body mechanics with reasonable accommodation if needed.
Visual, Hearing and Communication Requirements:
Requires corrected vision and hearing to normal range, with or without reasonable accommodation. Ability to communicate by way of the telephone with participants, customers, vendors and staff.
Pressure Factor:
Requires working under stressful conditions. Working conditions may be noisy and crowded and fluctuating indoor temperatures. Moderate pressure to meet scheduled deadlines. Potential for exposure to verbal aggression by client, vendors, and staff.
Environmental Conditions:
May be exposed to a risk of bodily injury through contact with moving instrumentation, toxic substances, medicinal preparations, bodily fluids, communicable diseases and any other conditions common in a healthcare environment. Subject to unpleasant odors.
Duties and Responsibilities:
- Participate as a member of the interdisciplinary team on an as needed basis.
- Carry out plan of care activities within the CNA scope of practice as directed.
- Assist participants and/or supervise with activities of daily living (bathing, dressing, grooming, hair care, mouth care, eating, hand and food nail care, and toileting) as assigned or needed under direct supervision.
- Create and participate in group(s) and one-on-one activities in accordance with daily activity calendar and as requested by the Recreational Coordinator.
- Monitor and assist participants with meals.
- Throughout the day, assist in maintaining clean and orderly day rooms, kitchen and personal care areas.
- Accompany enrollee to medical and other appointments.
- Perform and or assist with laundry, including clothes for participants
- Assist transportation staff with the loading and unloading of participants from vehicles.
- Provide Escort assistance on transportation vehicles when requested.
- Note changes in participant’s physical or emotional health and communicate this information to the Day Center Supervisor or designee.
- Maintain participant’s medical record and fulfill charting and reporting requirements.
- Assist the transportation department with participants’ takes home supplies.
- Act only within the scope of his or her authority to practice.
- Participate in and support Quality Improvement initiatives.
- Follow all Policies and Procedures and OSHA safety guidelines.
- Maintain the confidentiality of all company procedures, results and information about participants, clients or families.
- Handle potentially infectious specimens with appropriate biohazard precautions and practices standard precautions.
- Maintain safe working environment. Follow Safety Policies and Procedures
- Demonstrate dependability through consistent compliance with scheduled work hours.
- Participate in continuing education classes and any required staff and training meetings.
