Job Summary:
Under the direction of the Clinic Manager oversees the day-to-day operations of the Home Care Services program. Directs and supervises Homecare Nurses, Home Health Aides, and Home Care Scheduler. Assists leadership in policy formation, program development and implementation. Provides supervision visits to home care services staff, conducts initial home safety assessment and reassessments, and coordinates plan of care in collaboration with the Interdisciplinary Team. Provides skilled home care services for participants and health education for participants and caregivers. Acts as the liaison between [PACE Organization] and contracted home health agencies.
Qualifications and Requirements:
Education / Training / Certifications:
- BSN degree with an RN license
- BCLS Certification
- Current drivers license and proof of auto insurance
Experience:
- Minimum of 3 years of nursing management in a community home based setting preferred.
Skills and Knowledge:
- Knowledge of home care services nursing principles and practices with reference to the elderly.
- Broad knowledge base of health care problems of the frail elderly.
- Good assessment skills. Skilled in identifying problems and recommending solutions.
- Knowledge of common safety hazards in the home and improvements to establish a safer environment.
- Familiarity with community health resources available to participants and families
- Knowledge of current [State] home health policies
- Skilled in preparing and maintaining records and written reports and responding to correspondence.
- Excellent organization and documentation skills.
- Skilled in developing and maintaining department quality assurance.
- Skilled in establishing and maintaining effective working relationships with participants, medical staff, staff members and family caregivers.
- Ability to react calmly and effectively in emergency situations.
Medical Clearance:
Employees must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.

Work Environment:
Employee must be able to work effectively within an interdisciplinary team model, interfacing and collaborating with a wide range of clinical and social services disciplines who work together to manage the PACE participants’ care. The work setting is in an Adult Day Health Center and primary care clinic environment with moderate noise levels and controlled temperatures.
Physical Requirements:
Requires manual and finger dexterity and eye-hand coordination; the ability to use department equipment; and the ability to lift/carry up to 30 pounds using appropriate body mechanics with reasonable accommodation if needed.
Visual, Hearing and Communication Requirements:
Requires corrected vision and hearing to normal range, with or without reasonable accommodation. Ability to communicate by way of the telephone with participants, customers, vendors and staff.
Pressure Factor:
Requires working under stressful conditions. Working conditions may be noisy and crowded and fluctuating indoor temperatures. Moderate pressure to meet scheduled deadlines. Potential for exposure to verbal aggression by client, vendors, and staff.
Environmental Conditions:
May be exposed to a risk of bodily injury through contact with moving instrumentation, toxic substances, medicinal preparations, bodily fluids, communicable diseases and any other conditions common in a healthcare environment. Subject to unpleasant odors
Duties and Responsibilities:
- Demonstrate effective leadership in managing daily operations of the home care department and services.
- Perform initial assessments of participants in their home environment to determine individual needs, assess current support systems, safety of the home environment, and need for support services in the home.
- Perform scheduled and unscheduled re-assessments to assess home care service needs and provision of services as planned.
- Develop home care plan of care in coordination with the Interdisciplinary Team and caregiving resources in the home (family, friends, etc.).
Participate in the development of the participant’s plan of care at scheduled plan of care meetings. - Maintain regular attendance at, and participate in, interdisciplinary team meetings, communicating participant changes, collaborating on care planning decisions, and coordination of 24-hour care delivery.
- Provide orientation, initial and annual competencies, supervise, and evaluate performance of homecare nurses, home health aides, and home care scheduler.
- Perform quarterly home care supervisory visits of all home health staff
- Assure that home care staff provide skilled supportive and restorative nursing care to participants as prescribed in the home care plan of care.
- Assure that home care staff maintain participants’ medical records and fulfill charting and reporting requirements.
- Assure that home health nursing staff provide education and training to participants and their families on an individual or group basis as needed.
- Responsible for the coordination of home care services for all participants in the home.
- Serve as a liaison between [PACE Organization] and contracted home health agencies.
- Manage and prioritize utilization of homecare staffing needs to reflect enrollee homecare needs. Provide this information to the interdisciplinary team.
- Ensure all contracted home health agency staff complete initial and annual competency testing and fulfill all CMS and [PACE Organization] training and personnel requirements.
- Maintain utilization data for authorized home care services provided.
- Participate in developing and organizing new staff orientation, training and on-going in-service training for home care staff and independent contractors.
- Provide skilled nursing care in the home when needed.
- Participate in the on-call process.
- Follow all policies and procedures and OSHA safety guidelines.
- Maintain the confidentiality of all company procedures and information about participants, clients or families according to HIPAA regulations.
- Handle potentially infectious specimens with appropriate biohazard precautions, and practice standard precautions.
- Participate in and support Quality Improvement initiatives.
- Participate in continuing education classes and any required staff and training meetings. Maintain professional affiliations and any required certifications.
- Act only within the scope of his or her authority to practice.
- Perform other duties as required or requested.
