Job Summary:
Under the supervision of the Center Director, manages the daily, operations of the PACE Center. Responsibilities include the planning, coordination and implementation of all activities in the PACE Center. Works in conjunction with the Recreational Coordinator to assure that recreational activities are provided as scheduled. Assures that Day Center Workers complete assigned daily tasks.
Qualifications and Requirements:
Education:
- Associates Degree plus five (5) years related experience or Bachelors degree plus two (2) years related experience
- Valid State Drivers License
Experience:
Skills and Knowledge:
- Knowledge of principles, practices, standards and techniques of personal assistance services
- Working knowledge of healthcare and aging networks
- Demonstrated effective leadership and decision-making skills
- Effective supervisory skills
- Ability to foster collaborative working relationships
- Working knowledge of physical, mental, spiritual, transportation, and social needs of the frail elderly and their families.
- Working knowledge of quality improvement and cost containment systems.
- Ability to apply creative problem-solving skills
- Ability to effectively and efficiently plan, prioritize and follow-up on/delegate responsibilities
- Effective written and oral communication skills
- Working knowledge of local health care and geriatric service networks
Medical Clearance:
Employees must have medical clearance for communicable diseases and up-to-date immunizations, before having direct participant contact.
Competency:
Position specific competencies for the Day Center Supervisor will be met prior to assuming participant care.
Physical Demands and Working Conditions:
The physical demands and work environment characteristics described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Work Environment:
Employee must be able to work effectively within an interdisciplinary team model, interfacing and collaborating with a wide range of clinical and social services disciplines who work together to manage the PACE participants’ care. The work setting is in an Adult Day Health Center and primary care clinic environment with moderate noise levels and controlled temperatures.
Physical Requirements:
Requires manual and finger dexterity and eye-hand coordination; the ability to use department equipment; and the ability to lift/carry up to 30 pounds using appropriate body mechanics with reasonable accommodation if needed.
Visual, Hearing and Communication Requirements:
Requires corrected vision and hearing to normal range, with or without reasonable accommodation. Ability to communicate by way of the telephone with participants, customers, vendors and staff.
Pressure Factor:
Requires working under stressful conditions. Working conditions may be noisy and crowded and fluctuating indoor temperatures. Moderate pressure to meet scheduled deadlines. Potential for exposure to verbal aggression by client, vendors, and staff.
Environmental Conditions:
May be exposed to a risk of bodily injury through contact with moving instrumentation, toxic substances, medicinal preparations, bodily fluids, communicable diseases and any other conditions common in a healthcare environment. Subject to unpleasant odors.
Duties and Responsibilities:
- Directs, supervises, and evaluates the performance of the Day Center Workers.
- Assures that Day Center workers initial and annual competencies are scheduled, and completed within timeframes.
- Provides/coordinates in-services, and training for Day Center Workers according to program needs, and state and federal regulatory requirements.
- Ensures that the job responsibilities, authorities and accountability of all direct reports are defined and understood.
- Ensure that the daily assigned job responsibilities of the Day Center Workers are completed.
- In conjunction with the Center Director responsible for maintaining compliance with all state and federal program requirements, obtaining and maintaining certification under CMS as well as any other state or federal law or rules.
- Participate as a member of the interdisciplinary team, maintaining regular attendance at and participate in interdisciplinary team meetings; communicate participant changes, collaborate on care planning decisions and coordination of 24 hour care delivery.
- Actively communicate relevant issues to the team, ensures all team members are informed and up to date, participates in the development of the plan of care and monitors and articulates participant’s progress and decline.
- Maintain the PACE Center and its equipment as required by plant operations, and state and federal agencies.
- Monitor operations to ensure consistent compliance with established quality assurance standards.
- Recommend changes in service delivery and staffing as required by the development and growth of the agency.
- Participate regularly in all quality assurance/quality improvement committees as part of the overall quality improvement program.
- In conjunction with the Center Manager and Quality Assurance Coordinator maintain the stability and reputation of PACE by insuring that all activities and operations are performed in compliance with local, state, and federal contracts, regulations, laws, accreditation, protocol, licensing and certification requirements governing PACE operations.
- Monitors operational compliance with OSHA regulations and agency policies and procedures.
- Protect privacy and maintain confidentiality of all company procedures and information about, employees, participants, clients or families.
- Participate in continuing education classes and any required staff and training meetings. Maintain professional affiliations and any required certifications.
- Oversee coordination and scheduling of daily delivery of home delivered meals, DME, supplies and miscellaneous items as requested by the interdisciplinary team.
- Ensure that participant’s medical record and demographic information is maintained and updated.
- Perform related duties as required or requested.
