Medical Records Manager

Supervised by:
  • Quality Assurance Coordinator

Supervises:

  • None

Job Summary:

Under the supervision of the Quality Assurance Coordinator, prepares participant charts for new participants and other program needs. Sends for and releases medical information, compiles interdisciplinary assessment and treatment summaries, files, copies and faxes medical information.

Qualifications and Requirements:

Education:
  • High school diploma or equivalent.
Experience:
  • One year of experience in Medical Records
Skills and Knowledge:
  • Knowledge of medical records documentation requirements for health care setting.
  • Highly organized and accurate.   
  • Legible handwriting, knowledge of medical terminology, spelling skills and ability to enter data on computer.
  • Good organization skills, self-starter, demonstrates initiative.
  • Skilled in establishing and maintaining effective working relationships with patients, co-workers, medical staff, and the public.
  • Skilled in identifying and recommending problem resolution.
  • Ability to work independently with minimum supervision.
  • Ability to communicate clearly and effectively.

Work Environment:

Employee must be able to work effectively within an interdisciplinary team model, interfacing and collaborating with a wide range of clinical and social services disciplines who work together to manage the PACE participants’ care. The work setting is in an Adult Day Health Center and primary care clinic environment with moderate noise levels and controlled temperatures.

Physical Requirements:

Requires manual and finger dexterity and eye-hand coordination; the ability to use department equipment; and the ability to lift/carry up to 30 pounds using appropriate body mechanics with reasonable accommodation if needed.

Visual, Hearing and Communication Requirements:

Requires corrected vision and hearing to normal range, with or without reasonable accommodation. Ability to communicate by way of the telephone with participants, customers, vendors and staff.

Pressure Factor:

Requires working under stressful conditions. Working conditions may be noisy and crowded and fluctuating indoor temperatures. Moderate pressure to meet scheduled deadlines. Potential for exposure to verbal aggression by client, vendors, and staff.

Environmental Conditions:

May be exposed to a risk of bodily injury through contact with moving instrumentation, toxic substances, medicinal preparations, bodily fluids, communicable diseases and any other conditions common in a healthcare environment. Subject to unpleasant odors.

Duties and Responsibilities:

  • Prepares charts (electronic or paper) for new participants.  
  • Purges records and files documents into active and closed charts.  
  • Sends for medical records for PACE applicants at time of intake and as requested by the Interdisciplinary Team (IDT).
  • Acts as scribe for IDT morning meetings and POC meetings. 
  • Prepopulates minutes with carryover from previous day’s meeting.
  • Ensures Interdisciplinary team completes attendance sheet for daily IDT morning meeting and all POC meetings. 
  • Develops and updates schedule for periodic reassessments of PACE participants and communicates to the IDT.
  • Coordinates with Center Manager to develop schedule for Plan of Care (POC) meetings and communicates to the IDT 
  • Keeps face sheet information and participant emergency cards up-to-date. 
  • In coordination with the Quality Coordinator releases information according to HIPAA and Protected Health Information policies and procedures.
  • Scans all documents into EMR except forms faxed to clinic (labs, consult reports, discharge planning notes).
  • Maintains files for original signature documents (medical release forms, signature page of enrollment agreement, financial release forms, etc.)  
  • Assists Quality Manager in completing and sending grievance forms and appeal letters based on time based on regulatory guidelines.
  • Assists Quality Manager with data input of CMS audit universes.
  • Follows all PACE organization Policies and Procedures and OSHA safety guidelines.
  • Maintains the confidentiality of all company procedures, results, and information about participants, clients or families. Protects the confidentiality of all participant information.
  • Practices Standard precautions and follows appropriate Infection Control procedures.
  • Maintains safe working environment. Follows all PACE organization Safety Policies and Procedures.
  • Participates in and supports Quality Improvement initiatives.
  • Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.
  • Performs other duties as required or requested.
