HeritAgePACE provides clear information about who qualifies for the program and how individuals can enroll or disenroll. Participants must meet eligibility criteria, and all requirements are explained thoroughly during the intake process.
To enroll, individuals must complete an intake assessment, including a medical and functional evaluation. Once eligibility is confirmed, the participant signs an enrollment agreement and officially begins receiving services on the first day of the following month.
Disenrollment can occur voluntarily or due to relocation outside the service area, loss of eligibility, or other qualifying reasons. All transitions are supported to ensure continuity of care.
